Patient checklist

Before your initial consultation:

1. Call the clinic before your visit to make sure we have all your medical records. 
2. Plan to spend at least two hours in the clinic for your appointment. 
3. Bring your insurance card/information to your first appointment. 
4. Bring a list of all the medicines you are currently taking, or bring the bottles with you. 
5. It is often helpful to bring a friend or a family member with you.
